how to tick (Check) a value In checkboxlist in google doc form in edit mode

i have a google doc form and i want to select a value(tick or check) in google form doc. in edit mode how to do it i am a rookie pls tell me. image of the form attached.

currently i am trying to select a single check box i.e permanent as shown in the image but not able to do it need help. doc is in edit mode.everthing is editing but not the checkbox.let me know what i am doing wrong.1st time i am using the doc.
