How to modify checkbox value in html

I am trying to have a custom value returned when a checkbox is checked

< input type="checkbox" name="seizures" value="Seizures">Seizures/Tremors

I would like this form to return "Seizures" instead of "on" when checked, but the value command is not producing this desired result
