How to create checkbox and select data from Sql in asp.net webform?

I have a table in database[1st picture]. 1st webform shows all items name from the table with checkbox [2nd picture]. User can check the checkboxes and click the button to select. The checked items appear with textboxes. User can give values as quantity and they all will save in a new table in database[3rd picture].
