How save data of multiples checkbox in a same table(database)?

I have some checkbox's in a formulary, When i "checked" one or some of this checkbox, i need save this data in a database. I don't know if i must save in several tables or this data can be save in a single table in my database.

I need help only the logic of the database to save this data. E.G. the tables than i can use, etc.
