have some problem with options in opencart. Everything its ok, radio button working, select also but when i have last step checkbox, then this last checkbox not added to my card with product. What is wrong?

Click to add lenses -> then 3x click to next and then check some check box. Then click to thats all and product will be added to cart but without option.
