Monthly Checking of attendance using Checklist (PHP, MYSQL)

this is my very first time to ask here. I am creating a system of a church that needs a monthly checking of visits done to the members of the church. So I need to have the names of the members of the church and a checkbox right next to the names. I am a newbie of using PHP, and I think and hope you guys can help me to figure out how to do it.
