Saving checkbox state as checked or unchecked onto a servlet

I have a static checklist with associated checkbox items.enter image description here

When the user checks the checkbox, the state(checked) of the checkbox should be stored in Servlet. Initially all checkboxes are unchecked. I can't use the 'checked' property because it should come as unchecked on the page.
