how to populate Dynamic checkboxes and textbox for each checkbox side by side in html or css or javascript?

student_names = list of students, student_ID_numbers = list if student's ID number

i have a dynamic list of student names and ID numbers for those students. how to populate a dynamic checkbox for each student and a textbox beside to that checkbox to enter the ID number in HTML or CSS or javascript? I am trying to add this functionality in Jenkins "Active choices Reactive parameter"?please help me out.
