HTML creating and placing dynamic checkbox , I am not able to place in the required place on the form

I am building an HTML form using VBScript , I want to add checkbox to the form at specific location , I cannot pre create checkbox as the number of checkbox are dynamic , I am able to create checkbox but not able to place it to the right place on the form .
