ADD check box to nondatabase block

I need help about check box in oracle form 11g . I have database block execute the data then pressing push button that contains the code to send the data to Excel to make Report if I want to add check box (none database item )so that when I checked the line and pressing pushbutton it makes a report of the line which checked . I made the check box properties (when value checked=1 ,when value unchecked =0) ? Can you help me how to do it ?
