Advanced/Dynamic Checkbox

What I Need Is:

• Save the buttons onclick value;
• Tick First Checkbox;
• Append ?id=+name=""'s value only when 1 checkbox is ticked; Example: http://ift.tt/1hpxlkl
• If more is ticked append -+name="" Example: http://ift.tt/1OjLXyF
• If user unticks at any point remove its name="" from the onclick url.

I hope you see what I mean?
