Report generator based on user input obtained by radio button or check box

I am attempting to implement a report builder that generates a text paragraph based on input provided by the user. The user is presented 20 radio buttons or check boxes. Each selection is linked to a pre-defined sentence. Once the user selects however many of the radio buttons apply to them a paragraph is generated based on the selections.

I have built a sample of this in an Access database but would like to implement in python to give me more flexibility but having trouble figuring out where to start. I have basic python coding knowledge. Any advice would be greatly appreciated.
