pass values from checkbox to opened Form Page

i have a Message Form. In this Message Form the User click on a Adressbook Link and a little PopUp opens with the Recipient Addresses. The User can choice the Recipient as checkbox values. If the Users check a Recipient the Recipient value should pass to the Recipient Field in the Message Form. How and with Language (jquery, java script, etc.) can i do this function?

The User click Loupe Symbol the Addressbook open and the User chioce the Recipient. After the Choice the Recipient is in the Form Field left from the Loupe Symbol.
