Datas from UserForm to creat a dynamic table form [closed]

I would like to makeing a form kinda "To Do List" by UserForm given datas, but by tick or not a checkbox make the table dynamic. In the given excel i wrote what if then combination.

I couldnt make dynamic code still..please help me.
