Create dynamic checkbox via CSV file and javascript

I will like to build a web interface for this -->

I have a csv file to be generated as

(1) checkbox label , and upon user selection (select the checkbox)

(2) only for selected words : display with a text field

Then, export both information into a csv etc.

Any idea how to do it/ any tutorial for similar function? Thanks!
