PHP MySQL Approval page with checkboxes and dates they were checked.

I'm developing an internal PHP mySQL application for keeping track of product numbers. I've got most of it finished but we need an approval type page for each product (id) that simply has checkboxes, and will normally be false (0) until approved(1). What is the best way of setting this up both in the PHP and the database? Is there a way to track the date each box was checked? There will be 3.
