Is there a python libarary/ techniques to help to identify the wanted checkboxes from a pdf document with all boxes?

The idea is to select checkboxes for model to learn from. As in the image, those blue boxes are there for all fields, not just the checkboxes.

I want to have Yes checkbox, No checkbox, Male checkbox, female checkbox Email checkbox & Fax checkbox.

If anyone has done this before can share some of the experience, will be much appreciated.
