issues with checkboxes and when closing my database

I have a database which has several problems.

Item the first: I have some checkboxes in a report header. The checkboxes do not maintain their state when I requery or refresh the report. And when I check flow control the vba calls routines several times -- obviously I've done something silly but can't for the life of me catch it. I guess I've been looking at it too long, so I'm asking for fresh eyes on.

Item the second: when I close the report using the either Close Button, several routines also run prior to the window closing. I could be wrong but I don't think that is correct. I also suspect its related to the first problem in some fashion, so again can you'll look over the code and let me know what silly thing I've done?
