Adding Line Items and Check boxes by button

Item 1: I would like to be able to click this plus sign that will then add a checkbox that's similar to what Item 2 is pointing at.

Item 3: I would like to click the plus sign and it will create an original copy of the group within the pointers of Venue/Site.
